BOOMR® Project Acknowledged on the Minister of Health’s Honour Role
October 2015 – Medical Pharmacies has been acknowledged on the Minister of Health’s Honour Roll for our quality improvement work in the BOOMR® project. The Better Coordinated Cross-Sectoral Medication Reconciliation Initiative for LTC residents (BOOMR) set out to drive improvement in the medication reconciliation process through collaborative care across the CCAC, Royal Victoria Health Care Centre in Barrie, Woods Park Care Centre LTC, and its pharmacy provider, Medical Pharmacies. The selection process was very competitive this year, with an especially strong pool of 85 applications submitted from across the province. The Selection Committee was very impressed with the BOOMR application. As an Honour Roll application, the BOOMR initiative will be showcased at the Minister’s Medal booth at Health Quality Transformation (HQT) and highlighted on the ministry’s website. The BOOMR project has also been awarded a 2015 IDEAS Alumni Achievement Award as one of the five top QI projects of the past year. This award was presented by Ideas Advanced Learning from Heath Quality Ontario.
