Collaborative clinical support
Staff and residents receive the support they need through our board-certified geriatric Clinical Consultant Pharmacists (CCPs). Our team of CCPs support health professionals by providing drug information and quality indicators, participating in physician rounds and/or care conferences, medication reconciliation on admission, and comprehensive medication reviews. CCPs also support and encourage resident/patient and family involvement through personal consultation. The CCP fosters the best interests of residents by collaborating with physicians and nursing staff to ensure optimal resident care is achieved.
Clinical Consultant Pharmacists (CCPs) support nurses and staff at care facilities through regular educational in-services on various medication and disease topics, and regularly present to residents and families to keep them up-to-date on health and wellness issues of interest and importance. These in-services and information sessions are customized to the residence so that the material is relevant and better retained.
Reporting and audits
Medical Pharmacies offers robust, proprietary (home and pharmacy) medication incident software that allows both staff and pharmacies to complete and update incident reports. Our system allows users to add information as it becomes available, and the final report integrates investigations and analysis throughout the process. With over 50 standard reports available in our client-customized resource centre, we can also notify and escalate reporting at various levels in organizations. Dedicated, seamless transition
Switching pharmacies should not be a huge undertaking. Medical Pharmacies has perfected the transition process for hundreds of clients through a specialized, experienced transition team. With a dedicated project lead, you have a single point of contact throughout the transition process. Our aim is to minimize disruption for you and create a smooth, seamless transition for your residents.
eMAR: We work with eMAR systems from PointClickCare®, MEDeCare and Catalyst. Since investing in early technology in 2004, Medical Pharmacies has supported approximately 250 homes with over 29,000 residents, facilitating an introduction to electronic medication administration (eMAR), and integrated medication management (IMM) systems. The robust knowledge and expertise of our Information Technology and eMAR teams ensure that operators and staff get the most from your eMAR system.
Digital prescribing: Our digital prescribing technology, MEDeDigital®, allows care facilities to save time through its unique features such as digital order paper, pre-populated with resident name, allergies, and Creatinine Clearance (CrCl) values. All orders can be viewed or searched, by physician or resident, via the facility’s dedicated online portal.
PACMED & NIOSH labelling: We use industry-proven technology, combined with best practices from years of experience to provide the highest dispensing accuracy. For medications that require special handling (identified by the National Institute for Occupational Safety and Health – NIOSH), we separate these medications in the strip packaging, and specially label directly on the pouch to help distinguish NIOSH drugs. This supports safety and compliance for front line staff, in line with the Institute for Safe Medication Practices (ISMP).
Medical Pharmacies provides equipment to support our clients’ medication management system. This includes equipment such as medication carts, pill crushers, narcotic disposal units, touchscreen eMAR tablets, emergency boxes, glucometers, medication fridges & thermometers, sharps containers, and much more. If repair or maintenance is required on the equipment, our customer service centre consists of dedicated support professionals who can quickly, accurately, and professionally answer questions and troubleshoot issues for our clients.
BOOMR®: People are vulnerable to harmful medication errors, particularly during transitions from hospital or community to retirement or nursing homes, often due to lapses in communication, documentation or transcription. BOOMR (Better Coordinated Cross-Sectoral Medication Reconciliation) is a redesign of the medication reconciliation (MedRec) process to improve patient engagement, increase quality of information on admission and discharge, and create collaborative workflow efficiencies during transfer. The result is is a unique, pharmacist-led program that reduces hospital visits, unnecessary medications, and both hard and soft costs for care facilities, as well as saving valuable healthcare staff time, allowing them to focus on providing better care for residents.
Medical Cannabis: Like any pharmaceutical product or natural medicine, combining cannabis with other medications can have adverse effects. Medication management is a key component in maximizing the safety and efficacy of medical cannabis. Medical Pharmacies offers a unique medical cannabis program that ensures treatment is provided to your residents in the safest and most efficient manner. The program integrates with MAR/eMAR, which ensures oversight on all medications the patient is taking and prevents harmful interactions. Delivery through our pharmacies facilitates a single point of control. Our program includes self-directed eLearning for healthcare staff, and coordinates with a specialized clinic for prescriptions and support. Products are categorized by colour for ease of understanding and communicating product selection, and our Licensed Producer ensures product supply commitments to prevent interruptions to therapy.
